Union Community Health Center (UNION), operating continuously as a health care provider in the Bronx since 1909, is one of the largest Federally Qualified Health Centers (FQHC) in NYS. With a proven record of accomplishment, UNION provides a full range of quality primary care and dental services for children, adolescents, adults, and seniors in underserved, high-need communities throughout the Bronx.

UNION’s mission is to provide comprehensive, culturally competent primary and preventive health care services to improve the health status and decrease health disparities of the medically underserved populations in the Bronx, providing services to all regardless of age, race, ethnicity, sexual orientation, religion, or the ability to pay.

UNION continues to impact more residents each year with five clinic locations and two Mobile Units (medical and dental). The organization serves over 30,000 underserved residents of the Bronx and provides over 180,000 patient care visits annually. UNION is an impressive model of a holistic and integrated approach to patient care that unifies primary care, behavioral health, and dental care providers, creating one team dedicated to designing and delivering care for each patient. Additionally, UNION provides specialty services—including physical and occupational therapy, speech therapy, audiology, cardiology, behavioral health, and psychiatric services—that are often difficult for Medicaid patients to access.

Principal Mission

Under the direct supervision of the AVP of Pediatric Wellness, the Registered Dietitian plans, directs, and provides nutritional care to patients. This includes conducting nutritional assessments and providing nutrition education for patients and members of the medical team. Responsibilities include providing appropriate nutritional care for the age populations served (infant, pediatric, adolescent, adult, and geriatric).

Principal Duties and Responsibilities

1. Promote a safe, cooperative, and professional health care environment to ensure optimal patient care.
2. Adhere to all hospital policies, procedures, rules, and regulations, including but not limited to absenteeism, ETIME, cellular telephone usage, dress code, and rules of conduct.
3. Make Customer Service a priority, treating patients, visitors, and co-workers in a professional manner with courtesy and tact. Effectively communicate with patients and others about UCHC services and ensure concerns reach the right person(s) in a timely fashion.
4. Interview patients, families, and/or significant others to obtain diet history necessary for planning the patient’s diet.
5. Gather appropriate anthropometric, biochemical, and physical data necessary to assess patients’ nutritional status and document information accurately in the medical record.
6. Assess patients’ nutritional status based on standards of nutritional care and assign an appropriate level of nutritional risk.
7. Develop appropriate care plans based on standards of care, including recommending therapeutic diets or nutrition support.
8. Reassess patients based on level of nutritional risk or change in condition and modify the plan of care as necessary.
9. Collaborate with members of the health care team on a regular basis in developing and implementing treatment plans.
10. Assess patient/family/significant others’ educational needs, readiness to learn, and barriers to learning; provide education and document in the medical record.
11. Keep current in the field of nutrition through continuing education to maintain registration status through the Academy of Nutrition and Dietetics (75 Continuing Education Units/5-year period). Complete 3 CEUs annually in area of practice and 3 CEUs related to age-specific topics (pediatric, geriatric).
12. Demonstrate competency in assessing the nutritional needs of patients across the lifespan (pediatric, adult, and geriatric).
13. Perform all other duties as directed.
14. Conduct nutrition education classes for patients and staff as needed.
15. Function as a preceptor for dietetic intern students in areas of clinical and community nutrition and evaluate students’ progress.
16. Communicate regularly with the Chief Clinical Dietitian regarding patient care and operational issues.
17. Counsel patients/families on medical nutrition/diet therapy principles and restrictions.
18. Utilize available medical nutrition therapy education materials or obtain from reputable sources.
19. Document the nutrition counseling session in the Electronic Health Record.
20. Assist in education to increase patient referrals as needed.
21. Participate in mandatory in-services and meet any mandatory annual in-service requirements.

Qualifications

• Education Required: B.S. in Food and Nutrition / Dietetics
• License(s) Required: Registered Dietitian or Registry Eligible
• Experience/Skills Required: Two (2) years’ experience in Clinical Dietetics. Ability to speak Spanish is desired but not required. Strong ability to communicate with patients/family members and interact with other members of the healthcare team.

Working Conditions & Job Hazards

Variable from patient areas; may involve walking. Occasional exposure to patients’ illnesses. Normal office settings and some outreach services in the local community.
